Tumors shed DNA, cells, and other molecules into the bloodstream, and liquid biopsy — analyzing these components from a simple blood sample — has emerged as a powerful tool in oncology. By detecting circulating tumor DNA (ctDNA), liquid biopsy offers a minimally invasive window into a cancer genetic makeup and burden. This capability is transforming several aspects of cancer care and holds tantalizing promise for the holy grail of oncology: detecting cancer early, before symptoms appear.

Current Clinical Uses

Liquid biopsy already has established clinical roles. In advanced cancers, it can identify targetable mutations to guide therapy selection when tissue biopsy is difficult, and detect emerging resistance mutations during treatment. Monitoring ctDNA levels can reveal treatment response and detect recurrence earlier than imaging in some cases. After surgery, detecting residual ctDNA can identify patients at high risk of recurrence who might benefit from additional treatment, informing increasingly personalized decisions.

The Early Detection Frontier

The most anticipated application is multi-cancer early detection — blood tests that screen for many cancers simultaneously in people without symptoms. Several such tests are in development and study, aiming to catch cancers at earlier, more treatable stages. Significant questions remain about accuracy, the risk of false positives and overdiagnosis, and whether early detection through these tests actually improves survival. Large trials are underway to answer these questions. The technology is promising but its ultimate role is still being defined.
