Airway Management: Size Matters Critically
Pediatric airway anatomy differs fundamentally from adults: the narrowest point is the subglottic cricoid ring (not the larynx), making uncuffed ETTs appropriate for children under 8 years in most settings (though modern microCuff ETTs are appropriate from birth). ETT size estimation: (age/4) + 4 for uncuffed, (age/4) + 3.5 for cuffed. Oral airway sizing by placing along the mandible from incisors to angle of jaw. Standard catheter sizes by age: premature neonates (24–26g), term neonates/infants (22–24g), toddler/school-age (22–24g), adolescent (20–22g adult standard). SpO2 monitoring requires pediatric and neonatal-sized probes — adult probes applied to small fingers produce inaccurate readings from optical misalignment. Blood pressure cuff selection requires cuff bladder width covering 40% and length encircling 80–100% of upper arm circumference — an adult cuff on a toddler will produce falsely elevated readings. Neonatal blood pressure cuffs (sizes 1, 2, 3), infant cuffs (size 4), child cuffs (size 5), and small adult cuffs (size 6) should all be stocked in pediatric care areas.
