Stroke causes 140,000 US deaths annually and is the leading cause of long-term disability — with 87% being ischemic and 13% hemorrhagic. The good news: 80% of strokes are preventable through optimal management of modifiable risk factors. The 2021 AHA/ASA Stroke Prevention Guidelines and subsequent 2023 updates have refined risk stratification, anticoagulation thresholds, and blood pressure targets in ways that significantly impact clinical practice.
Atrial Fibrillation: The Dominant Modifiable Risk Factor
AFib carries a 5× increased stroke risk — accounting for 15–20% of all ischemic strokes and up to 30% of cryptogenic strokes. CHA₂DS₂-VASc scoring guides anticoagulation: score ≥2 in men, ≥3 in women → anticoagulation recommended (class I). Score of 1 in men, 2 in women → shared decision-making (class IIa). Direct oral anticoagulants (DOACs — apixaban, rivaroxaban, dabigatran, edoxaban) are preferred over warfarin for nonvalvular AFib — 19% lower stroke rate, 51% lower ICH rate, and comparable major bleeding in meta-analyses. Left atrial appendage occlusion (Watchman device): for patients with absolute DOAC contraindication — FDA-approved, with 5-year data showing non-inferiority to warfarin for stroke prevention.
Blood Pressure: The Most Scalable Intervention
SPRINT trial (SBP target <120 vs. <140 mmHg): 25% reduction in first stroke with intensive BP control in non-diabetic adults. Every 10 mmHg SBP reduction → 20% reduction in stroke risk in meta-analyses covering 1 million participants. Current AHA 2021 guidance: target SBP <130 mmHg for secondary prevention, <140 mmHg for primary prevention in most adults. ACCORD data (diabetic patients): intensive SBP control did not reduce MI but did reduce stroke — supporting treating BP aggressively in diabetic patients despite MACE benefit uncertainty.
